En effet, il s&rsquo;agit d&rsquo;une version abr\u00e9g\u00e9e de la <a href=\"https:\/\/www.psy.link\/la-therapie-dacceptation-et-dengagement\/\">Th\u00e9rapie d&rsquo;Acceptation et d&rsquo;Engagement (ACT)<\/a>, d\u00e9velopp\u00e9e par Hayes, Strosahl et Wilson en 1999 et 2012. ACT est une th\u00e9rapie bien \u00e9tablie mais avec des suivis nettement plus longs, entre huit et 12 s\u00e9ances.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Un mod\u00e8le unifi\u00e9 de la souffrance et de la r\u00e9silience humaines<\/h2>\n\n\n\n<p>La particularit\u00e9 de l\u2019ACT r\u00e9side dans son fondement scientifique. Celui-ci montre qu&rsquo;un nombre limit\u00e9 de processus mentaux expliquent \u00e0 la fois la souffrance et la vitalit\u00e9 humaines. ACT soutient explicitement que des d\u00e9ficits dans un ou plusieurs de ces processus fondamentaux peuvent expliquer toute souffrance humaine, quelle que soit sa forme. Cela signifie que l\u2019ACT est une approche transdiagnostique ; il ne s&rsquo;agit pas d&rsquo;un traitement sp\u00e9cifique \u00e0 certaines maladies, troubles ou conditions. Si un patient obtient le b\u00e9n\u00e9fice maximal en douze s\u00e9ances, c&rsquo;est un traitement efficace en termes de temps. En revanche, si le m\u00eame b\u00e9n\u00e9fice peut \u00eatre atteint en seulement quatre s\u00e9ances, utiliser douze s\u00e9ances ne serait pas consid\u00e9r\u00e9 comme efficace en termes de temps. L&rsquo;intervention br\u00e8ve&nbsp;dans Focused ACT&nbsp;se r\u00e9f\u00e8re g\u00e9n\u00e9ralement \u00e0 un protocole d&rsquo;une&nbsp;\u00e0 quatre&nbsp;s\u00e9ances. Malheureusement, les patients qui abandonnent le traitement ne b\u00e9n\u00e9ficieront pas des outils et techniques pr\u00e9vus pour les \u00e9tapes ult\u00e9rieures. Le nombre moyen de s\u00e9ances compl\u00e9t\u00e9es par un patient typique lors d&rsquo;un \u00e9pisode de th\u00e9rapie varie de quatre \u00e0 six. Une intervention br\u00e8ve devrait donc \u00eatre con\u00e7ue pour \u00eatre termin\u00e9e avant d&rsquo;atteindre ce point de rupture naturel. Dans cette d\u00e9finition, l\u2019intervention br\u00e8ve repr\u00e9sente en partie, l&rsquo;acceptation de la part du clinicien que le temps disponible pour aider un patient sera limit\u00e9, que le processus th\u00e9rapeutique doit \u00eatre dirig\u00e9 par le patient et que la mission du clinicien est d&rsquo;aider le patient \u00e0 r\u00e9aliser des changements comportementaux significatifs pendant le temps disponible.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">La d\u00e9tresse psychologique motive \u00e0 poursuivre la th\u00e9rapie<\/h2>\n\n\n\n<p>Comme nous l\u2019avons tous observ\u00e9, les patients entrent en th\u00e9rapie \u00e0 cause de niveaux \u00e9lev\u00e9s de d\u00e9tresse psychologique. Puis, \u00e0 mesure que leur d\u00e9tresse diminue, ils sont de moins en moins enclins \u00e0 revenir pour des s\u00e9ances suppl\u00e9mentaires. En effet, les principales motivations des patients \u00e0 chercher de l&rsquo;aide sont le r\u00e9confort \u00e9motionnel et la r\u00e9solution pratique des probl\u00e8mes. Ces deux r\u00e9sultats sont facilement atteints pour la plupart des patients au cours des premi\u00e8res s\u00e9ances de th\u00e9rapie. En r\u00e9alit\u00e9, la th\u00e9rapie est une contrainte pour la plupart des patients, n\u00e9cessitant temps et ressources pour y assister.<\/p>\n\n\n\n<p>Les recherches montrent que la plupart des patients mettent fin \u00e0 la th\u00e9rapie rapidement, avec ou sans l&rsquo;accord de leur th\u00e9rapeute. Environ 30 \u00e0 40 % des patients abandonnent le traitement sans consulter leur th\u00e9rapeute. Dans une \u00e9tude de plus de neuf mille patients en th\u00e9rapie, une grande majorit\u00e9 a mis fin au traitement avant la cinqui\u00e8me s\u00e9ance. Encore, le nombre modal de visites en psychoth\u00e9rapie \u00e9tait d&rsquo;une seule. Cependant, des recherches montrent que 50 % des gains totaux de la th\u00e9rapie sont obtenus avant la huiti\u00e8me s\u00e9ance. Apr\u00e8s la huiti\u00e8me s\u00e9ance, les progr\u00e8s ralentissent consid\u00e9rablement. Le nombre de s\u00e9ances de th\u00e9rapie n&rsquo;est pas un facteur pr\u00e9dictif significatif du changement clinique. Cela qui sugg\u00e8re que le changement est un processus non lin\u00e9aire. Les traitements brefs peuvent \u00eatre tout aussi efficaces que les traitements \u00e0 plus long terme. D\u2019ailleurs, des \u00e9tudes montrent que le degr\u00e9 de r\u00e9duction des sympt\u00f4mes et l&rsquo;am\u00e9lioration \u00e0 long terme du fonctionnement social sont tout aussi importants dans un traitement de huit s\u00e9ances que dans un traitement de seize s\u00e9ances. Les traitements brefs sont tout aussi efficaces que les traitements \u00e0 plus long terme pour divers troubles.<\/p>\n\n\n\n<p>L&rsquo;impact \u00e0 long terme de la psychoth\u00e9rapie sur le fonctionnement du patient est une question cl\u00e9. Les traitements qui se contentent de r\u00e9duire les sympt\u00f4mes transitoires de d\u00e9tresse cr\u00e9ent un probl\u00e8me \u00ab&nbsp;boomerang&nbsp;\u00bb. L\u2019intervention br\u00e8ve ciblant le changement comportemental afin de changer des patterns de comportements inadapt\u00e9s peut induire du mieux-\u00eatre. Les \u00e9tudes montrent que ces gains se maintiennent \u00e0 long terme.<br>D&rsquo;autres \u00e9tudes montrent que 40 \u00e0 45 % des patients d\u00e9prim\u00e9s enregistrent des progr\u00e8s soudains et importants au cours des deux \u00e0 quatre premi\u00e8res s\u00e9ances de traitement.
