J\u2019ai trop honte, rougir c\u2019est vraiment ridicule. Je me dis que je suis nulle. Je me sens terriblement d\u00e9courag\u00e9e. Je ne serai jamais normale&#8230; je n\u2019aurais jamais de nouveaux amis.&nbsp;\u00a0\u00bb <\/p>\n<\/blockquote>\n\n\n\n<p>Anna est une jeune patiente de 20 ans qui souffre de solitude li\u00e9e \u00e0 la difficult\u00e9 de lier des amiti\u00e9s. Elle d\u00e9crit bien son besoin de lien social mais aussi la fuite et l\u2019\u00e9vitement de nouvelles rencontres.&nbsp;Nous avons fait ensemble une analyse fonctionnelle pour comprendre le d\u00e9roulement d\u2019un situation type. Anna d\u00e9crit des pens\u00e9es et \u00e9motions qui la submergent lorsqu\u2019elle doit se pr\u00e9senter lors d\u2019un diner. Une peur de rougir, des id\u00e9es de jugement \u00ab&nbsp;je suis compl\u00e8tement incomp\u00e9tente, ils vont me prendre pour une idiote&nbsp;\u00bb. Elle prend conscience de cet enchainement \u00ab&nbsp;d\u2019exp\u00e9riences internes&nbsp;\u00bb qui se fait automatiquement, d\u2019\u00e9motions et pens\u00e9es.&nbsp;<\/p>\n\n\n\n<p>Nombreuses sont les personnes qui ne font pas attention a ce qu\u2019ils se disent (les pens\u00e9es) ou \u00e0 l\u2019\u00e9motion qui surgit. Pourtant les pens\u00e9es et \u00e9motions interagissent et s\u2019influencent mutuellement. Lorsque Anna se dit \u00ab&nbsp;je suis nulle&nbsp;\u00bb, cette pens\u00e9e nourrit la honte. Cela va des pens\u00e9es automatiques \u00e0 des pens\u00e9es tr\u00e8s \u00e9labor\u00e9es. De l\u2019id\u00e9e d\u2019aller \u00e0 la plage au concept abstrait de relativit\u00e9. Notre esprit peut en effet nous amener du monde concret dans le pr\u00e9sent \u00e0 la dimension abstraite de la r\u00e9alit\u00e9. A travers ces processus mentaux et cognitions nous \u00e9laborons des concepts, cr\u00e9ons de lien entre eux et produisons des nouvelles connaissances. Ce \u00ab&nbsp;mod\u00e8le mentale&nbsp;\u00bb est \u00e9galement un moyen d\u2019\u00e9conomie, puisque nous n\u2019avons pas besoin de reconstruire notre repr\u00e9sentation du monde continuellement. En effet, notre appareil cognitif est un complexe syst\u00e8me unique \u00e0 l\u2019\u00eatre humain.&nbsp;<\/p>\n\n\n\n<p>La capacit\u00e9 de repr\u00e9sentation du monde nous permet d\u2019anticiper ce qui va se passer. Par exemple, nous n\u2019avons pas besoin d\u2019atteindre l\u2019accident pour savoir que si nous ne tournons pas le volant, la voiture va s\u2019\u00e9craser contre le mur de la maison. Ou si nous voyons une personne entrer par la porte dans une pi\u00e8ce nous savons qu\u2019elle est \u00e0 l\u2019int\u00e9rieur. Nous pouvons aussi imaginer et ressentir la tristesse de notre enfant qui s\u2019est fait moqu\u00e9 par ses copains ou nous pleurons lorsque nous regardons un film triste car nous nous mettons \u00e0 la place des personnages.<\/p>\n\n\n\n<p>Notre exp\u00e9rience, la connaissance du monde, de nous-m\u00eames et des autres nous permettent d\u2019aller au-del\u00e0 du visible afin de construire l\u2019invisible. Il permet de verbaliser l\u2019exp\u00e9rience visible et exprimer nos ressentis et pens\u00e9es. Notre m\u00e9moire cr\u00e9\u00e9 des traces mn\u00e9siques de ce que nous percevons en interactions avec nos pens\u00e9es et puis nous mettons en rapport ces \u00e9l\u00e9ments. Ce m\u00e9moire s\u00e9mantique organis\u00e9 en r\u00e9seau a des connexions. Imaginez des n\u0153uds ou chaque n\u0153ud repr\u00e9sente un \u00e9l\u00e9ment pr\u00e9cis tel qu\u2019un mot, une \u00e9motion, une phrase, une image, une sensation etc.&nbsp;<\/p>\n\n\n\n<p>Les n\u0153uds qui partagent la m\u00eame \u00e9motion sont plus proches et plus homog\u00e8nes. Ainsi, les exp\u00e9riences de joie sont oppos\u00e9es \u00e0 celles de tristesse et les deux peuvent s\u2019inhiber. Par exemple, lorsque nous pensons \u00e0 une personne que nous aimons, nous ressentons des sensations de chaleurs, de la joie, on peut visualiser cette personne et peut-\u00eatre nous formulons dans nos pens\u00e9es \u00ab&nbsp;je l\u2019aime&nbsp;\u00bb. Elle est tr\u00e8s anxieuse dans des situations sociales. Elle a toujours \u00e9t\u00e9 timide. Les personnes inconnues activent \u00ab&nbsp;le n\u0153ud&nbsp;\u00bb \u00e9motionnel&nbsp;<em>anxi\u00e9t\u00e9<\/em>. Au coll\u00e8ge, Anna \u00e9tait victime de harc\u00e8lement et aujourd\u2019hui elle pense syst\u00e9matiquement qu\u2019elle ne sera pas accept\u00e9 par autres. Elle se voit comme compl\u00e8tement \u00ab&nbsp;nulle&nbsp;\u00bb en habilit\u00e9s sociales. Le regard de l\u2019autre est associ\u00e9 \u00e0 l\u2019id\u00e9e qu\u2019elle sera jug\u00e9e. Elle pense \u00eatre inadapt\u00e9e. Ces pens\u00e9es sont li\u00e9es \u00e0 l\u2019id\u00e9e de la honte et \u00e0 l\u2019anxi\u00e9t\u00e9. Ces \u00e9motions sont tr\u00e8s inconfortables et Anna a tendance \u00e0 vouloir \u00e9viter des situations qui pourraient engendrer cet inconfort.&nbsp;<\/p>\n\n\n\n<p>Ainsi, pour Anna, de nombreux \u00e9l\u00e9ments constituent son r\u00e9seau d\u2019anxi\u00e9t\u00e9 sociale. Elle n\u2019en est pas consciente mais plut\u00f4t victime d\u2019une activation syst\u00e9matique d\u2019anxi\u00e9t\u00e9, d\u2019images mentales, d&rsquo;\u00e9motions et de pens\u00e9es automatiques en lien avec la situation de rencontre avec des inconnus. Ces situations sont \u00ab&nbsp;l\u2019interrupteur&nbsp;\u00bb de la l\u2019anxi\u00e9t\u00e9. Anna a tendance \u00e0 \u00e9viter des situations o\u00f9 il y a des inconnus car elle veut \u00e9viter de souffrir d\u2019anxi\u00e9t\u00e9 et de honte.<\/p>\n\n\n\n<p>En effet, cette jeune femme semble pi\u00e9g\u00e9e. Le r\u00e9seau s\u00e9mantique d\u2019anxi\u00e9t\u00e9 sociale se r\u00e9p\u00e8te et l\u2019anxi\u00e9t\u00e9 se propage d\u00e8s qu\u2019un \u00e9v\u00e9nement li\u00e9 \u00e0 ce n\u0153ud est activ\u00e9. Cependant, elle souffre de plus en plus, car il y a une envie forte d\u2019avoir une vie sociale riche avec des amis proches. Les diff\u00e9rents \u00e9l\u00e9ments permettront de comprendre la probl\u00e9matique. La r\u00e9p\u00e9tition des comportements d\u2019\u00e9vitement ne fait que renforcer les sch\u00e9mas sous-jacents. Il faut cesser la r\u00e9p\u00e9tition car celle-ci est le composant central au cercle vicieux maintenant les difficult\u00e9s.<\/p>\n\n\n\n<p>Les pens\u00e9es automatiques d\u2019Anna \u00ab&nbsp;je suis nulle&nbsp;\u00bb, \u00ab&nbsp;je ne suis pas normale&nbsp;\u00bb, l\u2019emp\u00eachent de d\u00e9passer la peur puisqu\u2019elles la nourrissent. Plusieurs techniques permettent de rep\u00e9rer ces pens\u00e9es n\u00e9gatives et les rationnaliser. Enfin nous chercherons \u00e0 diminuer leur impact sur les \u00e9motions de la patiente. On dira aussi qu\u2019on cherchera \u00e0 affaiblir la connexion du r\u00e9seau s\u00e9mantique.&nbsp;<\/p>\n\n\n\n<p>En effet, lors d\u2019une th\u00e9rapie TCC, le th\u00e9rapeute aidera le patient \u00e0 \u00ab&nbsp;restructurer&nbsp;\u00bb ces pens\u00e9es dysfonctionnelles. Ces derni\u00e8res n\u2019aident en rien la patiente dans sa vie, au contraire, l\u2019emp\u00eachent plut\u00f4t de vivre la vie qu\u2019elle souhaiterait avoir.&nbsp;<\/p>\n\n\n\n<p>Une autre technique cognitive de distanciation par rapport aux pens\u00e9es provient de la th\u00e9rapie d\u2019acceptation et d\u2019engagement. Son but est de rationaliser et diminuer l\u2019impact afin d\u2019amener le patient \u00e0 consid\u00e9rer les pens\u00e9es comme des purs produits de notre esprit. La v\u00e9racit\u00e9 n\u2019est pas questionn\u00e9e mais leur utilit\u00e9. Est-il utile et aidant de penser \u00ab&nbsp;je suis nulle&nbsp;\u00bb&nbsp;?<\/p>\n\n\n\n<p>D\u2019autres techniques consistent \u00e0 progressivement aller \u00e0 l\u2019encontre de la peur. En s\u2019exposant, au lieu d\u2019\u00e9viter, la personne gagne petit \u00e0 petit en confiance. Cela revient \u00e0 cr\u00e9er des nouvelles connexions dans le r\u00e9seau. En effet, en affaiblissant certaines et en cr\u00e9ant et renfor\u00e7ant d\u2019autres, on arrive \u00e0 mettre en place des nouveaux comportements qui sont plus positifs pour la
