Elle n&rsquo;est pas directement li\u00e9e \u00e0 un \u00e9v\u00e9nement d\u00e9clencheur identifiable. Ses causes sont neurobiologiques, g\u00e9n\u00e9tiques et neurochimiques.<\/p>\n\n\n\n<p>Ce type de d\u00e9pression est parfois appel\u00e9&nbsp;<strong>d\u00e9pression m\u00e9lancolique<\/strong>&nbsp;dans les classifications diagnostiques modernes (DSM-5, CIM-11), ou encore d\u00e9pression biologique.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Les m\u00e9canismes biologiques en jeu<\/h3>\n\n\n\n<p>Plusieurs dysfonctionnements biologiques ont \u00e9t\u00e9 document\u00e9s dans la d\u00e9pression endog\u00e8ne :<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>D\u00e9r\u00e8glement des neurotransmetteurs :<\/strong>\u00a0une diminution de la s\u00e9rotonine, de la dopamine et de la noradr\u00e9naline est classiquement observ\u00e9e. Ces mol\u00e9cules jouent un r\u00f4le essentiel dans la r\u00e9gulation de l&rsquo;humeur, du plaisir et de la motivation (Belmaker &amp; Agam, 2008,\u00a0<em>New England Journal of Medicine<\/em>).<\/li>\n\n\n\n<li><strong>Hyperactivit\u00e9 de l&rsquo;axe HPA (hypothalamo-hypophyso-surr\u00e9nalien) :<\/strong>\u00a0les personnes souffrant de d\u00e9pression endog\u00e8ne pr\u00e9sentent souvent un taux de cortisol chroniquement \u00e9lev\u00e9, signe d&rsquo;un stress biologique persistant m\u00eame en l&rsquo;absence de stresseur externe (Gold &amp; Chrousos, 2002,\u00a0<em>European Journal of Endocrinology<\/em>).<\/li>\n\n\n\n<li><strong>Anomalies structurelles c\u00e9r\u00e9brales :<\/strong>\u00a0des \u00e9tudes d&rsquo;imagerie c\u00e9r\u00e9brale (IRM) montrent une r\u00e9duction du volume de l&rsquo;hippocampe \u2014 r\u00e9gion impliqu\u00e9e dans la m\u00e9moire et la r\u00e9gulation \u00e9motionnelle \u2014 chez les personnes d\u00e9pressives chroniques (Sheline et al., 2003,\u00a0<em>Biological Psychiatry<\/em>).<\/li>\n\n\n\n<li><strong>Facteurs g\u00e9n\u00e9tiques :<\/strong>\u00a0la d\u00e9pression majeure est h\u00e9rit\u00e9e \u00e0 environ\u00a0<strong>37 %<\/strong>\u00a0selon les \u00e9tudes sur des jumeaux (Sullivan et al., 2000,\u00a0<em>American Journal of Psychiatry<\/em>). Pourtant, depuis plusieurs mois, Marie se l\u00e8ve chaque matin avec une sensation de plomb dans la poitrine. Elle n&rsquo;arrive plus \u00e0 ressentir de joie en voyant ses enfants, pleure sans raison apparente, et se r\u00e9veille syst\u00e9matiquement \u00e0 3h du matin, incapable de se rendormir. Son m\u00e9decin exclut tout \u00e9v\u00e9nement d\u00e9clencheur. Son grand-p\u00e8re paternel avait lui aussi connu des \u00e9pisodes d\u00e9pressifs s\u00e9v\u00e8res. Marie pr\u00e9sente tr\u00e8s probablement une d\u00e9pression endog\u00e8ne.<\/p>\n<\/blockquote>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><strong>Exemple 2 \u2014 Julien, 28 ans, \u00e9tudiant.<\/strong>&nbsp;Depuis ses 20 ans, Julien traverse des \u00e9pisodes de d\u00e9pression profonde qui surviennent sans pr\u00e9venir, parfois m\u00eame pendant des p\u00e9riodes qu&rsquo;il d\u00e9crit lui-m\u00eame comme \u00ab les meilleures de sa vie \u00bb. Les antid\u00e9presseurs ont consid\u00e9rablement am\u00e9lior\u00e9 son \u00e9tat. Son psychiatre \u00e9voque un trouble d\u00e9pressif r\u00e9current \u00e0 composante biologique marqu\u00e9e.<\/p>\n<\/blockquote>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Qu&rsquo;est-ce que la d\u00e9pression exog\u00e8ne ?<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">D\u00e9finition<\/h3>\n\n\n\n<p>La d\u00e9pression exog\u00e8ne (du grec&nbsp;<em>exo<\/em>, \u00ab \u00e0 l&rsquo;ext\u00e9rieur \u00bb) est, \u00e0 l&rsquo;inverse, d\u00e9clench\u00e9e par des&nbsp;<strong>facteurs ext\u00e9rieurs \u00e0 l&rsquo;individu<\/strong>: un \u00e9v\u00e9nement de vie difficile, une perte, un traumatisme, une situation de stress prolong\u00e9e. Elle pleure en permanence, ne sort plus de chez elle, a perdu tout int\u00e9r\u00eat pour ses activit\u00e9s habituelles. Elle se sent coupable de ne pas avoir \u00e9t\u00e9 l\u00e0 au moment du d\u00e9c\u00e8s. Lorsqu&rsquo;une amie lui apporte des nouvelles joyeuses de ses enfants, Sophie peut sourire furtivement \u2014 elle reste r\u00e9active aux stimuli positifs. Sa d\u00e9pression r\u00e9actionnelle au deuil est nettement li\u00e9e \u00e0 cet \u00e9v\u00e9nement pr\u00e9cis.<\/p>\n<\/blockquote>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><strong>Exemple 4 \u2014 Thomas, 52 ans, manager.<\/strong>&nbsp;Apr\u00e8s une restructuration d&rsquo;entreprise qui lui co\u00fbte son poste apr\u00e8s 20 ans de carri\u00e8re, Thomas d\u00e9veloppe une d\u00e9pression s\u00e9v\u00e8re. Il se sent inutile, honteux, sans avenir. Il dort trop, mange mal, ne r\u00e9pond plus \u00e0 ses proches. Mais lorsqu&rsquo;il retrouve un ami proche lors d&rsquo;un d\u00eener, il r\u00e9ussit \u00e0 rire pendant quelques heures.
